DIRECT SUPPORT PROFESSIONAL OR CAREGIVER QUALIFICATIONS AND REQUIREMENTS
- CNA License with at least 1-year experience.
- High School/GED diploma with 3 or more years of practical in-home care working with individuals with disabilities.
- Excellent knowledge of emergency response and First Aid and CPR or current certification.
- Have good time management skills.
- Assist supported person with prescribed medications.
- Have a valid driver’s license, insurance and working vehicle.
- Must be at least 18 years of age.
- Must be literate in the English language.
JOB DESCRIPTION AND SKILLS
- Assist supported persons with ambulation and mobility around the house or outside the residence as appropriate.
- Assist with physical therapy exercises.
- Assist with shopping, banking, transportation as required to community activities and employment.
- Perform light housekeeping duties that the supported person cannot perform without assistance.
- Assist with personal grooming, if supported person requires assistance or requests assistance.
- Be a pleasant and supportive companion.
- Follow the ISP as outlined to ensure that the needs of the supported persons are met.
- Maintain daily records of activities for the supported person.
- Maintain a shift handover records.
- Keep accurate documentation and reports.
- Complete Mileage Log, Individual Mileage Reimbursement Program paperwork, Risk Assessment Tool and Behavioral Checklist.
- Plan and prepare meals with assistance from supported person when they are able.
- Possess patience and friendliness with excellent communication skills.
- Discuss and follow handbook instructions and performing a variety of tasks to help supported persons understand their rights.
- Be able to perform, follow-up and complete activity checklist.
- Provide care in a compassionate and professional manner.
- Address supported person as they would prefer to be addressed.
- Report incidents as they happen in a timely and accurate manner as outlined in the policies and procedures.
- In the event the Family Model Caregiver is not able to provide services due to personal emergency or illness, they shall contact Program Director/Coordinator two hours before shift starts.
- In the event the Family Model Caregiver is not able to provide services, GOWO will provide a backup Family Model Caregiver to render services necessary through respite.
- The job is demanding as you may have to be available for most of the day and week as back-up or on call in case of emergency or urgent situation.
- Ensure that the rights of supported persons and services are maintained at all times.
- Work with supported persons Individual Support Committee for optimum benefit of the supported persons.
